
The transaminases ASAT and ALAT are enzymes found in liver cells, but also in those of the heart, muscles, and kidneys. When these cells are damaged, the enzymes enter the blood and their levels increase. In children, an elevation detected in a blood test does not automatically indicate a serious illness, but it always requires interpretation adapted to the age and clinical context.
Transaminase Norms in Children: Different Values from Adults
The reference values used in laboratories are often calibrated for adults. A level considered normal in a 40-year-old man may already indicate an anomaly in a 6-year-old child, and vice versa. The thresholds vary according to age, sex from puberty, and sometimes according to the laboratory’s testing technique.
A common pitfall is to directly compare the child’s result to the ranges printed on the analysis report, which mostly correspond to adult norms. An ALAT level slightly above the adult upper limit may be physiological in an infant, while a level within the “normal” high range in an adolescent may sometimes warrant monitoring.
When elevated ASAT and ALAT in children are identified, the doctor systematically compares the result to the age, weight, and clinical condition before concluding anything.
Pediatric Metabolic Steatosis: A Rapidly Growing Cause

Competitors largely address viral hepatitis, alcohol, and hepatotoxic drugs. These causes remain relevant, but they mainly concern adults. In children, metabolic liver steatosis (MASLD) has become one of the most common liver conditions, driven by the rise in overweight and sedentary lifestyles among young populations.
The term MASLD (metabolic dysfunction-associated steatotic liver disease) replaces the former designation NAFLD. It refers to an accumulation of fat in the liver associated with metabolic syndrome, without alcohol consumption. In overweight or obese children, a moderate but prolonged elevation of ALAT over several months should lead to this diagnosis.
The crucial point: this steatosis often progresses without any visible symptoms. The child complains of nothing, shows no jaundice or abdominal pain. Only the blood test reveals liver damage. A specialized consultation (pediatrician or pediatric hepatologist) then confirms the diagnosis, often through ultrasound, and establishes a nutritional management plan combined with a resumption of physical activity.
Pediatric Medications and Mandatory Liver Monitoring
Some treatments prescribed in pediatrics require regular monitoring of transaminases. This monitoring exists because the child’s liver metabolizes medications differently than that of adults, with variable sensitivity depending on age and enzymatic maturation.
Among the situations that justify regular testing:
- Antiepileptic treatments, some of which (notably valproic acid) present a higher risk of hepatotoxicity in young children than in adults.
- Biotherapies and immunosuppressants used in chronic inflammatory diseases in children, which require liver assessments before initiation and then at regular intervals.
- Long-term antibiotics or systemic antifungals prescribed for certain complex infections, which can cause a transient elevation of transaminases.
An elevation during treatment does not necessarily lead to stopping the medication. The doctor evaluates the benefit-risk ratio: if the increase remains moderate and stable, the treatment can be maintained under close monitoring. However, a rapid increase exceeding several times the normal level necessitates urgent specialized advice.
ASAT/ALAT Ratio in Children: What the Ratio Indicates

Beyond the raw level, the ratio between ASAT and ALAT provides a diagnostic direction that general articles rarely mention in the pediatric context.
When ALAT levels are significantly dominant (ASAT/ALAT ratio less than 1), the damage is generally of pure hepatic origin: steatosis, early viral hepatitis, drug toxicity. This profile is the most common in children.
When ASAT levels dominate (ratio greater than 1), an extra-hepatic origin should be considered. Since ASAT are present in skeletal muscles and the heart, intense physical exertion or a myopathy can elevate ASAT without the liver being involved. In a sporty child returning from intense training, an isolated elevation of ASAT does not necessarily indicate a liver problem.
Complementary testing of CPK (creatine phosphokinase) helps clarify: if CPK are also elevated, a muscular origin is likely. This test is simple and can be performed on the same blood sample.
When to Seek Emergency Care for Elevated Transaminases in Children
Not every elevation of transaminases justifies an emergency consultation. The distinction relies on three concrete criteria:
- The magnitude of the elevation: a level greater than ten times the normal suggests acute hepatitis (viral, toxic, or autoimmune) and requires rapid management.
- Associated clinical signs: jaundice (yellowing of the skin or eyes), intense abdominal pain, repeated vomiting, sudden fatigue, or confusion point to severe liver damage.
- The speed of onset: a rapid rise over a few days is more concerning than a moderate elevation discovered during a routine check.
In the absence of symptoms and faced with a moderate elevation, the usual approach is to recheck the results a few weeks later. A persistent elevation beyond six months warrants a complete etiological workup, including viral serologies, autoimmune assessment, liver ultrasound, and sometimes a pediatric hepatology consultation.
A level that normalizes spontaneously at the next check usually corresponds to a transient cause: common viral infection, muscle exertion, occasional medication intake. In this case, no further exploration is necessary, but the treating physician remains the appropriate contact to decide on the monitoring suited to each situation.
